If your insurance is Blue Cross, you may take advantage
of this premium-based continuation coverage as of Nov. 1, 2009.
If you are enrolled in HIP, continuation coverage will
be available on Feb. 1, 2010.

here. Premium rates are listed on the form.
Your insurance carrier (HIP or Blue Cross) should notify you with details
on the new state law in the near future.
