Important Notice on Optical Benefit EOBs

Please make Explanations of Benefit (EOB) requests in writing at least six weeks prior to your flex account filing deadline. Otherwise the Welfare Fund cannot guarantee Optical Benefit EOBs will be received on time. This policy is necessary owing to the high volume of last minute EOB requests.
